Founded in 2020 by Jordan F & Michael H, Healthy Transition Community (HTC) has come a long way from its beginnings in Chicago, IL. When Jordan F first started out, his passion for helping the community through spreading knowledge on how to become healthier" drove him to, focus on creating this program. During their downtime in the pandemic, the founders created HTC. HTC offers you in-home training at an affordable price, that works with any schedule. We now serve customers all over the Chicagoland area and throughout the USA and are thrilled that we're able to turn our passion into our own online business.

HTC understands the disparity between people of color and low economic standing, in relation to nutrition & exercise due to institutional structures as well as a lack of knowledge. My name is Jordan Fisher and I am the co-founder of Healthy Transition Community LLC. I grew up in South Shore community & have been an athlete most of my life. During my youth I was a collegiate athlete and competed in Track and Field. Professionally I have been conducting personal training sessions for people since high school. My education background, I received my Masters in Kinesiology from Southern Illinois University. And I have been a physiologist in cardiac and pulmonary rehab for the past year and a half. Currently, I am an Exercise Physiologist (EP) that specialize in designing and administering therapy for elderly adults (50-90 years old) that have been afflicted with the following: stroke, traumatic brain injury (TBI), and spinal injuries. I have been vegan for about 4 years now. During my time at grad school I really took initiative to improve the way I eat. During my transition I became friends with Mike. He witnessed my transition first hand and we put our minds together to make a business out of my lifestyle to positively affect OUR communities’ relationship with diet and exercise.
I work within the college setting to promote diversity in recreational settings. My professional goals relate to a long history of marginalized people being unable to enjoy outdoor recreational activities. My dream job is obtaining the opportunity to be a facility manager or recreational director. My goal is to change childhood exposure to these activities and encourage African American role models to reveal these possibilities. I attended Southern Illinois to complete my Master’s in Recreation Administration. I also gained my Bachelor's in Sports Administration from Southern Illinois before continuing my academic career.
